Rates & Insurance

Insured clients are charged based on our existing contracted rate or negotiable fee with each particular insurance.

Payment is due at the time of service.

Arbor Counseling is able to accept many different insurance plans as well as working with clients who want to do self-pay.  Insurance providers who we accept include but are not limited to:

  • Medicaid

  • Regence, BlueCross and BlueShield

  • United HealthCare/Optum

  • UMR

  • Bishop/Clergy pay

  • SelectHealth

  • Public Employees Health Plan

  • University of Utah Health Plans

  • Cigna/ Evernorth

  • Deseret Mutual Benefits Administration (DMBA)

Not all of our therapists are providers (credentialed) with the same insurance companies, which means your plan may identify a clinician as being out-of-network, so please contact our office for more specific information. Additionally, it is the responsibility of clients to know their benefits. Each plan is unique, so please review yours carefully. Many plans require a deductible to be met before they will pay any amount of the bill. If you have questions about your plan, call the number on the back of your insurance card.

Please be aware that benefits verification or pre-approval is not a guarantee of payment from insurance. Our billing department verifies insurance benefits for our clients as a courtesy only. Clients are ultimately responsible for verifying their own insurance benefits before starting and/or continuing treatment, especially when claims are denied for some reason.

Any copayments or insurance unpaid balances become patient's responsibility and must be paid at the time services are rendered. Clients should contact their insurance to resolve any unpaid or partially paid claims to prevent receiving a large bill throughout the treatment.
